New Patient Registration

The practice welcomes new patients who live within our practice area.
We register Patients Monday- Thursday between 11am - 2pm. New patients will be required to provide two forms of identification, for adults one item of ID must be photographic the other proof of address(e.g. driving licence, passport, utility bill) When registering you will be invited to attend for a new patient registration medical. The purpose of this is to obtain details of your medical history and current medication whilst we are waiting for your records from your previous doctor.

If you require a Language, BSL Interpreter or a Deafblind Communicator the practice can arrange this for you.
